The Enrollment Process:

STEP 1           

  Complete and return application Form and Fees
   Review Financial Agreement and Policy
   Review Parent/Guardian Statement of Support
   Review Student Code of Conduct - Middle School

STEP 2          

   Entrance Testing - to determine academic/grade placement and/or acceptance.
NOTE: standardized testing taken in the past/current school year may be considered in place of entrance testing.
   Report Cards - CPCA needs the report card from previous year as well as last marking period of current year
   School Family Handbook - this manual needs to be read in preparation for the interview.

STEP 3        

        Family Interview - this will follow the placement testing and receipt of the student's
report cards and personal evaluation forms. The Registrar will contact you to set up an
appointment.

STEP 4          

   Upon notification of acceptance - all remaining fees are due
   Completion and Submission - of Financial Agreement; registration through
the Administrator’s Office
   Completion of Enrollment - student's file must be furnished with all items on this
Enrollment Checklist
   Transcripts from prior school
   Extended Care registration forms for students in grades K3-8th
   Copy of birth certificate and social security number
   Emergency forms

STEP 5

   Placement on Class Roster

STEP 6     

   Before the first day of school, the following items MUST be on file:
  Up-to-date immunization record
  Screening for lead poisoning
  Student health record
  Health history Parts I and II and physical examination forms completed for CPCA
  Proof of health insurance coverage
  Custody papers when applicable
